It is estimated that 1 out of 4 adults have hypertension and 7% of the adult population has diabetes. These conditions can be managed and complications can be avoided if there is early diagnosis and proper care\, including regular and consistent access to drugs and medicine. It is further estimated that half of those with these chronic conditions are not aware of the disease until complications arise. In some instances\, patients will suffer from a massive heart attack or stroke without knowing that they had elevated blood pressure and blood glucose levels for years.\n\nThe prevalence of hypertension and diabetes in the Philippines has created serious problems during the pandemic as patients with non-communicable co-morbidities were more likely to have more severe forms of COVID\, and sometimes this resulted in death. In short\, reflecting on the pandemic and how it impacted on the country\, we realized how underlying health risk factors create great vulnerability when new infections arise\, hence the importance of managing these non-communicable conditions early and with a high level of effectiveness.\n\nHypertension and diabetes can be managed in families and at the community level. However\, the importance of community-based medicine and family approaches has not been given enough emphasis\, Both conditions tend to be familial from a genetic perspective but also from a lifestyle perspective. Families that consume high levels of salt\, excess sugar\, and trans-fats are likely to have multiple members who may suffer from these two conditions without knowing it. Sedentary lifestyles can also be seen as patterns within families and communities that do not have safe spaces to walk or engage in outdoor activity. Hypertension and diabetes are family and community problems that also have family- and community-based solutions.\n\nIn this episode of STOP C.O.V.I.D. DEATHS\, we will take a closer look at innovations in early diagnosis\, management\, and education about hypertension and diabetes through community-based programs. We will also discuss the elements that make a community-based program successful and the critical role of local governments and other stakeholders in this process.\n\nThe main presenters are both Consultants with the Dept. of Family and Community Medicine at the UP Philippine General Hospital\, Dr. Anthony G. H.
