The paradigm of modern medicine is currently navigating a pivotal transition from a reactive, curative-focused framework toward a proactive, preventive health ecosystem. For decades, the Philippine healthcare landscape has predominantly functioned under a “break-fix” model, where clinical intervention occurs primarily at the manifestation of acute pathology. However, to achieve sustainable Universal Health Care (UHC) and mitigate the burgeoning burden of non-communicable diseases (NCDs), a systemic realignment is imperative.

Central to this transformation is the “Yaman ng Kalusugan Program,” more colloquially recognized as the YAKAP Program. This initiative represents a sophisticated evolution in health financing and service delivery, moving beyond simple inpatient subsidies to a comprehensive primary care benefit package. In integrating predictive health surveillance and early diagnostic interventions, the YAKAP Program seeks to institutionalize preventive behavior at the grassroots level. In this week’s webinar, we will discuss how this program utilizes clinical pathways and risk-stratification to identify vulnerabilities before they escalate into high-acuity, high-cost medical emergencies. The discourse will further dissect the intricacies of health systems strengthening by emphasizing the role of digital health integration and the longitudinal monitoring of patient outcomes.
